Mailing in your device for repair from the comfort of your home.


This form is for retail customers. Business customers, please use the Business Service Request.

1. Complete this form in its entirety.
2. One of our representatives will contact you within one (1) business day to confirm the repair and provide you with a prepaid shipping label.
3. Once confirmed, carefully package your device and battery. Omit items such as SD, SIM card and accessories. Then drop it off at your nearest USPS location.
4. Most in-stock repair items will be completed the same business day after receipt and returned via USPS Priority Mail.
